I hereby waive all claims, which I ma have against Norwest Deaf Outdoors Club and its directors, officers, volunteers and representatives from all liability for injury, deaf, accident, property damage or any other loss sustained by me as a result o my participation membership that may happen to me, spouse/partner, my children, associates and pets. ^*

 I agree to give my permission for Norwest Deaf Outdoors Club (NDOC) to use the image of the photograph and video of me and my children, associate and pets for publicity purpose to promote in NDOC’s website, online and social media. ^*
